A. David Matian, D.O.
As an osteopathic physician trained in both traditional and osteopathic physical medicine, Dr. Matian's passion since childhood has been to serve his community and his neighbors. As a former Chief Resident at U.C. Davis and a U.C.L.A. graduate, he remains current with the latest medical advances and guidelines to serve you the best ways he can.
Jacky Kourkchian, P.A.
Jacklyn Kourkchian is a board certified Physician Assistant with multiple years of experience in family medicine. She received her undergraduate degree from UC Irvine and her master's degree in Physician Assistant Studies from Touro University Nevada. Jacklyn is a compassionate provider who is strong advocate of preventive medicine and integrating the whole body approach during patient treatment.
Rachel Cho, F.N.P.
Rachel Cho is a Family Nurse Practitioner who is
passionate about providing quality care to all her patients. She earned a B.A. in Public Health from UC Berkeley before earning her Masters degree in Nursing from CSULB, where she received extensive didactic and clinical training with diverse populations in various settings. She is specifically interested in health promotion and disease prevention, as well as acute and chronic disease management.
Toni-Rose is currently a student at Cal State Channel Islands. Before joining our team, she volunteered as a Community Outreach Prevention and Education (COPE) Health Scholar/Leader at Adventist Health Glendale (AHG). AHG is a 515-bed hospital built on the Seventh-day Adventist faith and mission to improve the health of communities and to share God’s love by promoting healing and wellness for the whole person. COPE Health Scholars, a division of COPE Health Solutions, provides an educational opportunity for individuals interested in making a difference in health care, advancing their careers, and building lifelong relationships and learning through the COPE Connect network. When she's not busy working and studying, Tone-Rose enjoys her hobbies which are dance and vocals.
Cynthia Perez, Finance Coordinator
Cynthia has struck a difficult balance few have achieved - a work-home balance. When she's not in the office, she's hiking long trails, working out at the gym, or baking with her young son. During her training in Healthcare Finance, she received both Academic Excellence and Perfect Attendance awards. Her future professional goals include bridging from the administrative side of healthcare into the clinical realm. For now, her background, experience, and calm, cool nature give her all the tools she needs to successfully address challenging issues healthcare finance.
Alissa Macademia, Referral Coordinator
Alissa attended CSUN and majored in business management. She hopes to contribute to poverty-stricken neighborhoods by starting an agency to provide healthy and affordable food choices for low-income families. On her free time, Alissa enjoys working out and traveling (and so far she can cross off Japan, Mexico, Hawaii, England and the Philippines off her bucket list!).
Venessa Montenegro, Medical Assistant
Vanessa is a Certified Medical Assistant. Her future plans include becoming a registered nurse. Her hobbies include make up artistry and spending quality time with her children.
Jasmine Gonzales, Medical Assistant
Jasmine is a Certified Medical Assistant. She routinely impresses upon her patients her warmth, grace, and caring spirit. She aspires to further expand her knowledge in the medical field and become a Registered Nurse whereby she can contribute to the community at large. Outside of the office, she enjoys her other roles as a devoted mother and partner. In her down time she enjoys spending quality time with her daughter and family.
Colleen Rivas, Medical Assistant
Colleen’s future goals entail working for the World Health Organization and traveling to countries with little to no resources. She hopes to help populations with severe health problems find solutions to improve the country's overall physical, mental and social well-being. A major goal for her is to help eradicate terminal illnesses. While earning her bachelor’s degree at UC Merced she Created and launched a video explaining how to navigate the U.S. healthcare system and was published twice in local publications for her efforts in the Turlock Journal and Merced County Times. While she’s not busy with her professional talents, she enjoys her hobbies of weight-training and painting.
Natalie is currently studying sociology at Los Angeles Mission College with a long term goal of becoming a grade school child counselor. She brings us many years of experience in customer service and busy retail store front management. When Natalie is not working, she enjoys working out and spending time with her family.

Prime Care Physicians is a primary care medical practice started in response to a need for quality primary care in the local community. Our goal is to preserve the quality of the personal relationship between patient and doctor, prevent disease, and help guide our patients through the medical maze of specialists and consultations should a medical concern arise. Our long term goals are to expand in geographic locations served and services available.
